As Sardinia is an island, you can reach Cagliari in one of two ways: by air or by sea.

Meridiana is a Sardinian airline connecting
Cagliari with many Italian cities, except Rome. Meridiana has an agreement
with Alitalia.
There are many Meridiana
flights from Milan Malpensa hub. These flights can be used in connection
with Alitalia, Continental, and NorthWest flights to Milan Malpensa.

Coming to Cagliari by ferry requires having or renting a car. In this way, however, you can vist the beauties of Sardinia. In summer, there are many ferry connections to Sardinia. However, most of them are to and from the ports of North Sardinia, where most tourists spend their holidays and where beautiful Costa Smeralda is located (the Cagliari zone is also beautiful, but it is not as well known to tourists). It is the biggest and cheapest, but it is government-owned and its ships are crowded. It also operates high-speed ferries (40 knots).
It is the most expensive, but the trip is like a cruise.
